We will try to match any legitimate quote for similar services.

Our mission is to assist as many couples as possible achieve their fertility goals |
1. Initial Consult. Our infertility specialists are usually available for initial consultations within 1 week. At the initial consult we try to provide patients with as much information as possible regarding diagnostic fertility tests that are indicated, specific treatment options and your fertility prognosis. Many insurance companies will pay for the initial consult (co-pay may be required). The fee for the initial consult varies ($74 - $444) based on several variables including, the amount of time spent with the patient, the amount of time spent with the husband or spent reviewing the husband's medical information, amount of time spent reviewing the wife's prior medical records, the extent of the chart review required and completed, and whether the patient has had previous infertility treatment by a physician. A summary dictation will be sent to you and to your referring physician if desired. New consults with a fertility specialist are available Mon - Sat in our Sandy office.
Completing the patient portal questionnaire and obtaining all pertinent medical records in advance will allow our physicians to have access to all your records prior to your arrival. This will enable us to provide the best advice possible for your particular medical situation during your initial consultation.
If you do not have insurance coverage, please inquire about our pre-paid cash discount for initial consultation with one of our board certified fertility specialists (currently $56-$333). A limited number of free consults are available to couples who have a combined income of less than $45,000/year and limited assets. Please complete the appropriate application (see below under income based discounts) and submit it to our office manager, Michele Richards, for review and approval.
Initial consults are available by phone ($130 cash pre-paid fee must be paid by credit card at the time of scheduling). Insurance does not pay for phone consults but this can be very cost effective, especially for those who live several hours away.
One of our infertility specialists is usually available for initial consultation within one week of your request (if not you will receive a coupon for credit equivalent to the pre-paid cash value of the consult that can be applied to the cost of the consult if you are self pay or if you have insurance it can be applied to future uncovered services).
2. Follow-up appointments and consultations at our infertility clinic. We are usually available for follow-up consultations within 1 week. At the follow-up visit we try to provide patients with as much information as possible regarding diagnostic test results they have completed, specific treatment options recommended and when appropriate their updated fertility prognosis. Many insurance companies will pay for the follow-up consults and appointments (co-pay may be required).
The fee for follow-up in-office appointments varies from $80 - $254 (a prepaid cash discount price of $60 - $190 is available) depending on several variables including the complexity of the discussion, the amount of time spent with the patient and time spent reviewing records. Income based discounts are available - see below for further information and application.
If applicable, please ensure that all recommended tests have been completed and then allow 1-2 weeks for the reports to be available prior to the follow-up visit. This will enable us to provide the best advice possible for your particular medical situation.
Follow-up visits are available by phone (cost $53-$159 and require payment by credit card at the time of scheduling). There is usually no charge to answer a simple question ( if less than 2-3 minutes). Insurance does not pay for phone consults or follow-up discussions by phone but this can still be very cost effective for the patient, especially those who live several hours away. Please call the office regarding current fees.
We respect and value your time and strive to see patients in a timely manner. We will try to notify you of any delays in the schedule. If we are unable to see you within 20 minutes of the time of your scheduled appointment, we will provide a $50 coupon for future credit for any uncovered services.
3. Insurance. Our billing staff will be happy to evaluate your situation, and work with your insurance company as applicable. Many insurance companies in the Utah area cover the initial evaluation and diagnostic testing. We will be happy to bill your insurance company for services when appropriate. Regretfully, many insurance plans in the Utah region do not cover infertility treatments such as intrauterine insemination or in-vitro fertilization but our office will be happy to work with you to determine your best options. Please see the information about Reproductive Care Associates, PC (RCA), Reproductive Care Center, PC (RCC) and Reproductive Care, PC (RC) above as there are differences in the insurance preferred provider status for these different corporations.
Insurance assistance can also be obtained through a service called Fertility LifeLines. They will personally call your insurance company to verify your benefits for both infertility treatments and medications. Call 1-866-Lets Try or 1-866-538-7879 Monday - Friday 8 am - 12 am EST and Saturday and Sunday 8 am - 6 pm EST. See www.fertilitylifelines.com

5. Credit Cards. We accept most credit cards as payment for services. The cards we accept include VISA, MasterCard, Discover, and American Express.
There is a 3% surcharge for use of a credit card for charges above $5,000.
6. 100% IVF Money Back Guarantee. Due to the unique nature of infertility treatment and the obvious and easily definable endpoint (delivery of a baby), we offer a 100% money back guarantee plan for IVF treatment for couples that qualify. We have designed the program so that more than 90% of our patients who need IVF can qualify for participation. It includes up to 8 treatment cycles (4 fresh and 4 frozen). Please call our billing office for current estimated pricing. Additional premiums may apply in certain circumstances. It does not cover the cost of medications or anesthesia services. Our fees for the IVF procedures will be 100% refunded if you complete the program and do not deliver a baby. Please review the contract information with the billing staff for further details. A 100% IVF Money Back Guarantee program is also available for patients using an egg donor that includes up to 8 treatment cycles (4 fresh and 4 frozen). Please call the billing office for details. Currently more than 90% of our patients who participate in the 100% IVF Money Back Guarantee program successfully deliver a baby.
7. Global Single Fresh Conventional IVF Cycle package. We offer very competitive global discounted fee for service like in-vitro fertilization (IVF) and an egg donor cycle if the treatment cycle fee is paid in advance. We plan to provide a global fee package for other treatments such as intrauterine insemination (IUI) options in the near future.
Global Single Fresh Lower Cost Lower
Stimulation Cycle package. We now offer a lower cost, lower stimulation IVF option for $4,995 if the treatment
cycle fee is paid in advance. If extra monitoring is required or more than 4 oocytes are retrieved then additional fees may apply. This protocol requires much less monitoring
and significantly less medication (lower cost). The pregnancy rates are slightly lower than the conventional IVF package and usually extra embryos are not available for freezing. This is an excellent low cost option for many of our patients. Please arrange a new consultation so that this option can be discussed in detail if interested.
8. Global Multiple Fresh IVF Cycle packages. We offer
a global pre-paid discounted fee for multiple IVF cycles that include any
associated frozen embryo transfer cycles. A two cycle ,
three cycle, and four cycle package is available.
Contract is completed when pregnancy and live delivery is accomplished or all
purchased treatment cycles are completed, whichever comes first. Medications
and anesthesia costs are not included. See billing for complete
details and contract. This is an excellent option for patients who may not qualify for our 100% money back guarantee program or for those who prefer a lower cost option while maximizing the change for pregnancy with the option for multiple IVF attempts if needed.
9. Income based discounts. Couples who have a combined gross income less than $60,000 can apply for a graduated discount on services.
10. Fertility medications. One of the greater expenses of fertility treatment involves fertility drugs. Medications can cost up to $5,000 or more for each conventional IVF cycle
(average $2,000 - $3,000). The cost for medications for our low stimulation IVF option is usually less than $1,000. Our nurses have extensive experience in helping patients obtain the lowest price possible for legitimate, legal medications and can offer advice regarding the various gonadotropin options available. RCC currently offers one of the best prices in the United States for Bravelle (FSH). Currently the price is $45 per 75 IU vial (subject to change without notice). The medication is dispensed through one of our collaborating pharmacies such as Roxsan or The Apothecary Shops.

There is also a pharmacy in Logan (Cache Valley Pharmacy) that provides infertility medications at low cost. Phil Cowley is the pharmacist. Their phone number is (435) 787-1212. Since some of the medications are compounded they may not be subject currently to the usual FDA regulations and quality control procedures.
Some Fertility medication grants and discounts are also available.

There are several pharmacies that specialize in national delivery of fertility medications. The Apothecary shops (www.theapothecaryshop.com/Fertility.html ), Freedom Drug (www.freedomdrug.com) and IVPCare (www.IVPCare.com) will also help patients determine their insurance benefits for medications that are used during fertility treatment cycles.
Freedom Drug usually provides the best prices on EMD Serono™ infertility products. They also have excellent teaching material for the administration of most fertility medications. See their drug administration video by following the link.
Injection training materials are also available through our Web site using the Patient Portal (please call for details regarding access if needed).

Some patients obtain their medications from overseas pharmacies from Canada, Mexico, England (such as http://www.ivfmeds.com) and other locations. The legality of this process is debatable and we do not usually recommend sources from out of this country.

11.
Financing. Infertility treatment outcomes are rarely guaranteed (except in programs like shared risk) and thus as a general rule we discourage couples from incurring heavy debt to pay for treatment cycles. However, we realize that having children or expanding families is a high priority for many of our patients. For those that believe that financing their treatment is the best option, we have applications available and can help facilitate obtaining loans and financing.
Zions Bank believes that financial barriers should not stand in the way of your goals and the surgical procedures that you would like or need. They have different loans to accommodate the different needs of patients such as; home equity lines of credit, home equity loans, secured personal loans, unsecured personal loans, and revolving credit lines such as credit cards.
To determine the right option for you to finance your elective medical treatment go to www.zionsbank.com, call Zions Bank Teleloan at 1-800-510-4598, or visit any one of their many branches.
Reproductive Care, PC has several financing options for patients requiring IVF. Please check with
billing regarding details and availability. Up to 80% financing is available for the 100% money back guarantee and multiple cycle discount package programs for qualified patients. Up to 50% financing is available for single IVF treatment cycle fees.
12. Flex Plan (FP) or Flexible Spending Account (FSA). FP and FSA are benefit package options developed to provide you with flexibility and choice with coverage and spending on health care including infertility treatment. These plans are typically available to all permanent employees of a company. Many companies permit you to review your benefit choices each year to ensure that they continue to meet your personal circumstances. These plans can be very attractive because they are tax sheltered. The FSA dollars are withdrawn from your paycheck before taxes. For more information see IRS code IRC §105 and §213 or your company's human resources department.
13. National IVF Scholarship Program. The International Council on Infertility Information Dissemination (INCIID) sponsors this program. All taxpaying United States residents who meet the criteria are eligible to apply. You must be a registered INCIID Community member at the Bronze or higher level (cost has been approximately $55 annually). There must be a documented need for the procedure. Preference is given to couples with no living children. There must be documented financial need (usually combined income of less than $60,000). Applicants can have no insurance coverage for IVF.
When you come to Reproductive Care Associates, PC (RCA) or the Reproductive Care Center, PC (RCC), you will be receiving care from three of the most highly trained, most experienced, board certified infertility specialists (Reproductive Endocrinologists) in the Utah and Intermountain region. In addition to training in OB/GYN, a reproductive endocrinologist undergoes 2-3 years of additional specialized training. There are currently less than 900 board certified infertility specialists in the United States. (see http://www.socrei.org/).
